Eligible Dependents

Dependents of enrolled DriveTime employees are eligible to participate in the plan but are limited to the following:

- Spouses

- Common-Law Spouses (TX only)

- Same-sex Domestic Partners

- Children and Step-Children (in residence)

- Children and Step-Children of Domestic Partners (same-sex partnerships only)

- Any dependent specified by a Qualified Mandatory Court Support Order

We verify the eligibility of all dependents covered on our Medical, Dental, and Vision plans.

Documentation for all newly covered dependents must be faxed to 888-505-7130 prior to your benefits effective date or your dependents will be removed from coverage. Enrolling an ineligible dependent on your plan could result in termination of your employment.

Expanded Definition of Dependent - Same Sex Partnerships

DriveTime will now offer our medical plans, dental plans, vision plan and life insurance to same-sex domestic partner dependents. If you and your partner would like to participate in the Wellness Program, you must complete the required steps to comply and are subject to the same biometric pillars, including no tobacco use.

In order to cover your same sex domestic partner, you must complete an Affadavit of Domestic Partnership form, available below. In addition to completing this form, you must also submit evidence of dependent eligibility. Please visit the Benefits website to view a list of accepted supporting documents. Domestic partnerships will only be accepted for same-sex couples at this time. Please note, any premium contributions made by DriveTime on behalf of a non-dependent partner are considered taxable income.
