Family Status Change

A Family Status Change (FSC) provides an opportunity for you to make changes due to a qualified life event. The Family Status change needs to be submitted within 31 DAYS of the life event. If this deadline is missed the next opportunity to enroll is during the annual Open Enrollment.

Qualified Life Events include the following:

- Birth or Adoption of a Child
- Marriage or Divorce
- Loss of Other Coverage/Enrollment in Other Coverage

To update your benefits under a Family Status Change, complete the Family Status Change Form and submit with the required documentation to the HR Portal at 888-505-7130.
