Biometric Testing and online Personal Health Profile - PHP must
be completed within 60 days of your Medical coverage start date.
Your Biometric Information must also be updated in your WellCall
Personal Health Profile to calculate your Wellness Tier.
Annual Physical must be completed by DECEMBER 1, 2012
All Participants
Biometric Testing and Personal Health Profile (PHP) are due
November 15, 2011. Your Biometric Information is due in your
Personal Health Profile no later than November 30, 2011 to
calculate your Wellness Tier for 2012.
Annual Physical must be completed by December 1, 2011.
