Beneficiary Information
You have the opportunity to designate life insurance beneficiaries
for each of your eligible life insurance plans. These designations
can be made for all your life insurance plans.
Beneficiary information for all benefit programs is maintained
confidentially. In the event of your death, DriveTime will produce
the last beneficiary designation on record to the life insurance
company.
Beneficiary Designations
You should designate a beneficiary(ies) to receive life
insurance payments in the event of your death. Your
beneficiary(ies) can be one or more individuals, a trust, an estate
or a charity. If your primary beneficiary(ies) predeceases you, the
beneficiary's share will be divided equally among surviving primary
beneficiary(ies). You may also name a contingent beneficiary(ies)
to receive benefits if no primary beneficiary survives you.
Primary and contingent sums must each equal 100 percent on all
plans.

Filing a Claim
To file a claim for basic, supplemental, or dependent life
insurance benefits, you, your DriveTime department, or members of
your family should contact the DriveTime Benefits Department by
emailing benefits@drivetime.com.
The Benefit Department will provide the necessary forms and help
you or your beneficiary file a claim for insurance. A claim must be
filed within 365 days of the loss.
